*Conditions of Admission * 1. Only those who are committed to following the Lord Jesus Christ are accepted as students. 2. Applicants are selected for admission on the basis of spiritual, educational, personal, and financial factors. Should an accepted student later be deemed to be inadequately fulfilling the stated expectations of the institution, he/she will be so informed. Such a student may be requested to withdraw. 3. In accepting an offer of admission to Doxa the student indicates that it is his/her intention to abide by the regulations and community standards set by the program. 4. I give permission to Doxa to use photos/video/images taken of me during the program events, classes, and student activities to be used for promotional and communication purposes. I waive any and all rights to these images. 5. All Doxa and Accrediting College staff members, faculty members, and student leaders who have access to student files through the Populi network are bound by provincial privacy laws. Specifically, this means that no one (including friends and family members) can be given any personal information (eg. contact, financial, or academic information) about any individual within the Populi data base except that individual. No personal information should be printed from the Populi data base unless requested by that individual. Confidentiality also applies to all conversations about personal information learned through access to Populi data. Declaration: I declare that the information provided in this application is accurate and true. I have read and understand the Doxa's privacy policy concerning the collection, use and release of personal information. I have read and am in agreement with CA Church's Statement of Faith. I agree to abide by the principles and practices as outlined in the Leadership Agreement (at top of application) herein indicated.
Please arrange to have official transcripts sent, from your high school and any post-secondary institutions you have attended, directly to CA Church (2601 Spuraway Ave, Coquitlam). All physical transcripts must be sealed and unopened to be considered official. If your institution is able to e-mail official transcripts, they must send it directly to hello@cachurch.ca to be considered official.
If you are currently in high school, ask your school administration to send an interim grade report. Upon completion, an official transcript must be requested. Grade 12 or equivalent (G.E.D.) is required for anyone to earn a degree.
